Your vitamin D is very low, so you should start the supplements for it and also just consult online so that we can know the pattern of your hair fall and accordingly the treatment can be planned for you

Please do not worry. Effective treatment options are available; however, management depends on the severity and extent of the condition. A detailed medical history is essential for appropriate evaluation and treatment planning. Kindly schedule an online consultation with a dermatologist for a comprehensive assessment, prescription, and further guidance.”

Your report shows low ferritin and low vitamin D, and your B12 is just below the normal range. These are well known contributors to hair fall, even when your haemoglobin itself looks fine. Your thyroid levels are normal, but hair fall alongside irregular periods is worth checking from a hormonal angle too.
Next Steps
Correcting ferritin, vitamin D and B12 usually needs supplementation guided by your doctor, not a fixed over-the-counter dose. Please see a gynaecologist for a hormone panel and pelvic ultrasound to understand why your cycles are irregular.
Health Tips
Avoid starting high-dose supplements on your own, since the right dose depends on how low your stores actually are.
