Not serious, but you need to change your lifestyle modifications and dietary habits strictly. Shoulder pain due to hyperacidity .so take more fibrous diet to improve your symptoms as well.
Burning pain between the shoulder blades that worsens on stretching is more suggestive of a muscular/postural or thoracic-spine problem than acidity or fatty liver. Fatty liver usually causes no pain or, occasionally, discomfort under the right ribs—not isolated upper-back burning. Reflux typically causes burning behind the breastbone or upper-abdominal symptoms.
Next Steps
Since it has persisted for 3 months, consult a physician/orthopaedic doctor within a few days for examination of the neck, thoracic spine, shoulder blades and nerves; physiotherapy may be advised. Avoid prolonged poor posture and self-medication with frequent painkillers
Health Tips
Visit the emergency department immediately if pain is associated with chest pain, breathlessness, sweating, weakness or numbness of the limbs, difficulty walking, fever, unexplained weight loss, or loss of bladder/bowel control.
