I don't know why she is shying away from consulting a General Surgeon in her area. This type of Tele-consultation can land the doctor into Medico-legal problems . Please avoid such consultations.

Anal sex is one of the causes of developing anal fissure.Too much pressure, tight anal sphincter muscles, and poor blood supply to the anus may lead to their development and poor healing.
Next Steps
The goal of anal fissure treatment is to lower the pressure on the anal canal by making stools soft, and to ease discomfort and bleeding. Conservative treatments are tried first and include one or more of the following: Preventing constipation through the use of stool softeners, drinking more fluids while avoiding caffeine-containing products. Soaking in a warm bath (also called a sitz bath), 10 to 20 minutes several times a day, to help relax the anal muscles; apply anobliss cream twice daily

She had developed anal fissure due to anal sex. In fissure blood is very mild and passing stool is very painful.  Sometimes there is just streak of blood on the feces. There is always a fresh red colored blood in form of drops
