Greetings,  sorry about the pain.  You seem to suffer from flexibility issues at lower back and hips.
Next Steps
get to see a physio with knowledge about trigger point therapy and posture correction.  in 3 sessions your problem will be solved 70% and once pain is down, physio will teach you exercises which will help loosen tight muscles and strengthen the core.  it is best to hire a physio as your trainer for couple of months to learn and get trained for your back issues and also improve fitness as well

• Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Anterior tilt and slouching are two different things. Anterior tilt at 20's usually doesn't cause pain in sitting. An anterior tilt usually causes pain in standing. You seem to have weakness of all your stabilisers as you have a habit of slouching since childhood. Sit straight, do not slouch & do not take any back support. I know you cannot sit straight for more than 5 mins. So take support of hands & sit straight. For anterior tilt correction you can try pelvic bridging with a straight back. Good luck
