This treatment is being suggested on bases of the information provided. However I would like to examine & investigate the  patient in detail. -Control  Hypertension, Diabetes or  metabolic disorder, if there is any. --. Sleep on a hard bed with soft bedding. --. Keep no  pillow under the head. --. Avoid painful acts & activities. --. Fomentation with warm water/ Gel pac . Dear, MRI reports do suggest a severe spinal stenosis but the treatment prospects depends upon the symptoms of the patient.
Next Steps
Hot fomentation SWD/TENS therapy, physiotherapy Avoid cross leg sitting, squatting, heavy weight lifting. Muscle relaxant tablet such as Zerodol-TH4 1BD

If short duration pain, then medicine and physiotherapy for 6 week If long duration pain, then surgery is treatment.
