Uncontrolled bloodpressure

I'm using blood pressure medication from last one year initially i used to take telmisartan 40mg trio with amlodipine and hydrochlorothiazide but I check every even using that medicine there is 160/105 bp is there my physician recently changed that medicine to metoprolol, telmisartan and cilindipine combo METOSARTAN LN50 even after that also I detected my bp is 160/104 like that even when I stop taking medication for one dose it shows the same value, i suspected about bp machine but while checking others it shows normal values for non hypertensive patients what is going on i couldn't imagine, please tell the probable reason I'm also having diabetes using insulin twice a day mean while when I started using bp medication my hb level is decreasing from 15 to now 11 in a year and my sr creatinine levels also increased from 0.8 to 1.6 what is the probable cause for all this changes.. How can I survive with this

Measure the BP DAILY TWICELY  FOR 5DAY BY MANUAL BP MONITOR IF STILL HIGH THEN WE DO SOME MORE BLOOD TEST AUR CHANGE THE MEDICATION.   IF IT BELOW 140/90 THEN  NO NEED TO CHANGE THEN CONTINUE SAME MED.
