Hi,
I have attached the MRI diagnostic report. Currently he doesn't have much pain, but he gets imbalanced if he tries doing any activity like jogging etc.
As of the doctors are asking him to get operated sooner. My question is can it be pushed to 2 months to observe if thete is any improvement or is it better to get it done asap.
Or is there an alternative treatment for surgery which can strengthen muscles.
Kindly suggest sooner.

Arthroscopic acl repair is treatment of choice as soon as possible , as arthritic changes has already started suggesting long duration of injury.. you have already waited for long now. Don't worry for surgery much. These days advanced equipments are there for its repair
Next Steps
should go for surgery to resume your sports activity sooner
In view of instability its first important to do physiotherapy as from what you are saying the muscles appear weak.
The patient will need surgery at some time as there is a complete rupture of the acl ligament..acl ligament surgery is never an emergency surgery unless there is a fracture associated with it..which is not the case here..so physio first followed by reevaluation and then plan for surgery later.
Pain is not the concern. The instability of the joint is worrying.
You can wait for however long you want, but don't expect any improvement. As the knee continues to move in an abnormal manner further cartilage damage will occur.
So, It is better to get the surgery done as soon as possible
