Guyon’s canal syndrome occurs at the wrist. There is entrapment of the Ulnar nerve at the wrist in the guyon’s canal (ulnar canal) due to repetitive compression, prolonged wrist flexion, trauma, swelling, fractures, and vascular pathologies,that is commonly caused by direct pressure on a bike or cycle handlebar, weightlifting, excessive gripping, twisting, or repetitive wrist & hand movement.High-performance athletes and professional cyclist/cricketers/golfers/weightlifters can develop Guyon’s canal syndrome.  A hamate bone in the wrist sometimes fractures when a golfers club hits the ground instead of the golf ball and when cricket players are batting and bats hit the ground. And a fractured hamate bone can pinch the nerve inside Guyon’s canal. Symptoms of Guyon’s canal syndrome are- tingling/numbness/ in the finger 4 & 5, weak grip strength,pain, and sensitivity on the ulnar side (inner) of the forearm, wrist &hand muscle atrophy, clawing of digits 4 & 5. Physical Therapy Management /Interventions. Latest technology used for pain management- shockwave therapy, Tacar therapy, High-intensity laser therapy, combo therapy, etc. Other tools used are- Dry needling, Kinesio taping, etc. Instrument assisted Forearm release technique. Wrist flexor massage- with your elbow bent and palm facing up place a massage ball on a tender spot of your forearm flex your wrist up and compress the ball and slowly extend the wrist down. This can be repeated with a straight arm as well. Ulnar Nerve Flossing.Ulnar nerve gliding- two times/day and hold for 30 seconds for 3repetitions with a one minute break in between each repetition. Tendon gliding. Strengthening the grip and wrist muscles. Wrist muscles strengthening- Wrist extension with holding dumbbell & elbow bend &supported over the table. Forearm pronation & supination with dumbbell& elbow bend & supported. Tyler twist with Rubber bar (flexbar). Eccentric wrist extension and flexion. Wrist splints: splints keep the wrist in the neutral position and prevent it from bending. Use them during sleep.
