As a dermatologist, few things are more rewarding than seeing a patient leave the clinic with renewed confidence in their appearance. This is the story of one such patient — a case of vitiligo on the upper lip that had persisted despite conventional treatment, and how a simple surgical procedure made a meaningful difference.

What Is Vitiligo and Why Does It Appear on the Lips?

Vitiligo is a common acquired skin condition in which certain areas of the skin lose their natural pigment, resulting in white or depigmented patches. This happens because the immune system mistakenly attacks melanocytes — the cells responsible for producing skin color. While vitiligo can appear anywhere on the body, the face and particularly the area around the mouth are among the most frequently affected regions. Triggers such as repeated friction, minor skin injuries, and stress can initiate or worsen the condition in genetically susceptible individuals.

For many patients, vitiligo on the upper lip carries a significant emotional and cosmetic burden. It is one of the first things people notice, and the impact on self-esteem can be profound.

When Surgery Becomes the Right Option

Most patients with vitiligo begin treatment with topical medications and phototherapy, which work well in many cases. However, when a patch is localized, stable, and has not responded adequately to medical management, surgical repigmentation becomes a highly effective next step.

The key word here is stability. Before recommending any surgical procedure, I assess whether the vitiligo has remained unchanged  no new patches, no expansion of existing ones  for a sustained period. Stable vitiligo responds significantly better to surgery, and the likelihood of lasting repigmentation is much higher when the disease is not actively spreading.

This patient presented with a well-defined, stable depigmented patch on the upper lip  making them a suitable candidate for surgical intervention.

The Procedure : Suction Blister Epidermal Grafting

The technique I performed is called Suction Blister Epidermal Grafting, or SBEG  one of the most reliable and minimally invasive vitiligo surgery options available for small, localized lesions on cosmetically sensitive areas. The procedure involves creating a controlled suction blister on a concealed donor site, typically the inner thigh or buttock. The thin roof of this blister contains a rich layer of pigment bearing epidermal cells. This is carefully harvested and transferred onto the depigmented patch on the upper lip, which has been gently prepared under local anesthesia using light dermabrasion to create a receptive surface. The graft is then secured with a non-adherent dressing to allow undisturbed healing. 

The entire procedure is performed in the clinic under local anesthesia. It is well tolerated, requires no hospitalization, and uses the patient's own skin  making it a safe and natural approach to restoring pigment.

Recovery and Aftercare

Post-procedure care is straightforward. Patients are advised to keep the dressing intact, avoid rubbing or stretching the lip area during the initial healing phase, and attend regular follow-up appointments to monitor graft take and pigment development. Once re-epithelialization is complete, I typically introduce topical tacrolimus and targeted phototherapy to stimulate melanocyte activity and support natural color blending into the surrounding skin.

Re pigmentation is a gradual process. Patients begin to notice visible color returning within four to six weeks, with full blending and maturation continuing over three to six months.

The Outcome

This patient's follow-up photographs tell the story clearly. There is visible, natural re pigmentation of the upper lip with an excellent color match to the surrounding skin. The graft took well, pigment has spread evenly, and the patient is delighted with the result.

 Outcomes like this reinforce why careful patient selection and precise surgical technique make all the difference in vitiligo management.
