Introduction:-The gut microbiome is a well-recognized factor in the pathophysiology of obesity. This review highlights the mechanisms by which gut dysbiosis can contribute to the pathophysiology of obesity. In particular, we discuss gut microbiota’s contribution to host energy homeostatic changes, chronic low grade inflammation and bile acid metabolism via bacterial metabolites, such as short-chain fatty acids, and bacterial components, such as lipopolysaccharides, among others. Finally, therapeutic strategies based on next-generation probiotics aiming to re-shape the intestinal microbiota and reverse metabolic alterations associated with obesity are described. obesity is associated with chronic low grade inflammation. As a result, a person with obesity experiences decreased immune tolerance to antigens, which increases the chances of allergies and other immune diseases.
Chronic: A symptom or condition that persists even with treatment.
Idiopathic: The symptom or condition occurs for no known reason. It comes on suddenly.
Urticaria: The medical term for hives, pronounced ur-tik-CARE-ee-uh. urticaria is a common skin condition characterized by the development of wheals (hives), angioedema.
A case Report:- A 49 year old male, patient id-JFJ001665, IPD-2023/101 from United kingdom got admitted with C/O-Severe allergic rashes all over the body and itching and swelling, urgency of defecation immediate after food, retro-sternal burning sensation, and obesity, Abd Girth 126CM, weight-117 kg, height 5'11 feet, BMI-36-obesity since 2014-15.all investigations are enclosed. He was on Antihistamine and steroids' medication since 2014-15.
After 7days Rx And Virechan- Abd Girth - 104 cm, Weight-105.5kg
Signs and Symptoms of Urticaria:-Batches of welts (wheals) that can arise anywhere on the body Welts that might be red, purple or skin-colored, depending on your skin color Welts that vary in size, change shape, and appear and fade repeatedly Itchiness (pruritus), which can be intense Painful swelling (angioedema) around the eyes, cheeks or lips Flares triggered by heat, exercise or stress Symptoms that persist for more than six weeks and recur often and anytime, sometimes for months or years.
Chronic Idiopathic Urticaria Linked to Obesity:-H1 receptor antihistamines such as cetirizine, fexofenadine, and desloratadine are among the most commonly prescribed medications for the treatment of allergies and have been shown to stimulate appetite and weight gain and leads to obesity.
Role of Virechan in-Chronic Idiopathic Urticaria Linked to Obesity.
Virechan:-Virechana is one of the Panchakarma therapies wherein purgation is induced by drugs and it specifically aims at the elimination of excessive Pitta Dosha from the body through the anal route.
Virechana is effective in the management of the obesity as it helps to reduce colonization of aerobic bacteria -correction of the gut flora dysbiosis, and expel/drains out the extra cellular and intra cellular fluid which was due to low grade inflammation thus initiated the weight loss mechanism in the body, resulting in diminution in the signs and symptoms of obesity. Virechana Karma clears the Srotoavarodha (obstruction), normalizes the Agni and brings balanced state of Tridosha. Gastrointestinal (GI) disorders often accompanied by skin manifestations. Exactly how the gut microbiome is related to skin inflammation and influences the pathophysiology mechanism of skin disorders .The associations between the gut microbiome with inflammatory skin disorders, such as acne, psoriasis, atopic dermatitis, and urticaria, so here virechan karma helps to clean the GI microbiome from intestines and helps to reduces both Urticaria linked to obesity.

Haridra (Curcuma longa L) as the main ingredient that has been proven to have anti-inflammatory (preventing the intestines from microbes manifestation) and antiallergic properties by inhibiting Immunoglobulin E-mediated Mast Cells activation.
