Introduction
Tooth pain can be sudden, sharp, and debilitating affecting sleep, eating, and overall well-being. At Summirow Dental, we recently treated a male patient who came in with severe pain in the lower right back tooth (tooth 46) that had intensified over the past two days. Upon diagnosis, we found irreversible pulpitis, a condition requiring immediate endodontic intervention. Fortunately, with advancements in dental technology, we were able to restore his comfort through a single-sitting root canal treatment (RCT).
Patient Complaint
The patient presented with:
- Severe, continuous pain in the lower right jaw

Clinical Examination & Diagnosis
A thorough clinical and radiographic examination revealed:
- A deep carious lesion in tooth 46 (lower right first molar)
- No visible swelling, but the tooth was tender to percussion
- Positive response to pulp vitality testing (lingering pain)
- The radiograph showed a large cavity approaching the pulp with no significant periapical changes

Given the clear diagnosis and the urgency of the patient’s pain, we decided to proceed with a single-sitting root canal treatment to remove the infection, alleviate pain, and preserve the tooth.
Treatment Procedure
- Anaesthesia: Local anaesthesia was administered to ensure a pain-free procedure.
- Isolation: The tooth was isolated using a rubber dam to maintain a sterile environment.
- Access Opening: An access cavity was prepared to reach the infected pulp tissue.
- Canal Cleaning & Shaping: All root canals were cleaned, shaped, and disinfected using rotary endodontic files and advanced irrigants.
- Obturation: The canals were sealed using gutta-percha and root canal sealer.
- Temporary Restoration: A composite build-up was placed to seal the tooth until the final crown is planned.
Post-Treatment Instructions
The patient was prescribed painkillers and advised on:
- Eating soft foods for a day or two
- Avoiding biting hard on the treated tooth
- Returning for a permanent crown to fully restore the function and strength of the molar
