Patient Profile
A patient in his 30s came to Summirow Dental with severe gum recession, bleeding gums, and persistent bad breath. The patient reported difficulty maintaining oral hygiene and noted increasing tooth mobility.
Preoperative Assessment
Clinical Examination:
Periodontal Probing Depths: Probing depths ranged from 5 to 8 mm in the affected areas, with significant bleeding on probing.Clinical Attachment Loss: Notable attachment loss in the upper anterior region and the 1st and 4th quadrants. X-rays showed bone loss, particularly in the regions of interest.
Diagnosis:
Chronic Periodontitis - Severe attachment loss and periodontal pockets in the upper anterior region, as well as in the 1st (upper right) and 4th (lower right) quadrants.
Treatment :
Step 1: Scaling and Root Planing (SRP)
The treatment began with a comprehensive SRP, where plaque, tartar, and bacteria were removed from below the gumline. This procedure helped reduce inflammation and prepared the gums for healing. We also provided the patient with advice on improving her oral hygiene.
Step 2: Re-evaluation and Surgical Intervention
During the follow-up, some improvement was seen, but deeper pockets remained in the upper anterior region and 1st and 4th quadrants. We recommended flap surgery for the upper anterior region and LANAP (Laser-Assisted New Attachment Procedure) for the other quadrants.
Step 3: Flap Surgery
A full-thickness flap was elevated in the upper anterior region to access deeper pockets. Scaling and root planing were performed directly on the roots before repositioning and suturing the gum flap.
Step 4: LANAP Procedure
LANAP was used for the 1st and 4th quadrants, targeting infected tissue with a specialized laser. This procedure promoted healing, reduced inflammation, and encouraged new attachment formation with minimal discomfort.
Post-Operative Care and Outcomes:
We closely monitor our patients' recovery after periodontal treatments to ensure the best possible results. Here’s a look at the post-operative care and outcomes following our patient’s flap surgery and LANAP treatment. Pain Management and Oral Hygiene Pain Management. After the procedure, the patient was prescribed analgesics as needed. The discomfort was generally mild and was effectively managed with over-the-counter medications.
Oral Hygiene: We provided detailed instructions on maintaining oral hygiene using a soft-bristled toothbrush and antimicrobial rinses to avoid irritation and support healing.
Dietary Recommendations: A soft diet was recommended for the initial healing period to reduce stress on the gums and promote recovery.
Follow-Up Appointments:
1 Week Post-Surgery: During the first follow-up, we assessed the healing process, removed the sutures from the flap surgery, and checked for any signs of infection. The patient reported moderate discomfort but no significant issues.
6 Weeks Post-Surgery: At the six-week mark, we evaluated the surgical sites and assessed pocket depths. Significant improvements were noted in pocket depths and tissue health, and the LANAP-treated areas showed reduced inflammation and better attachment levels.
Outcomes:
Flap Surgery: Pocket depths in the upper anterior region were reduced, and gum health improved. Bleeding on probing decreased significantly.
LANAP: The LANAP treatment led to a significant reduction in pocket depths and inflammation in the 1st and 4th quadrants, enhancing tissue healing and regeneration.
