Chief Complaint:The patient, presented to our clinic with a history of a traumatic dental injury that occurred approximately 8 years ago. The incident involved a forceful blow to the face, resulting in significant trauma to the upper front teeth.

Description of the Incident:The patient vividly recalled the incident, narrating that it occurred during a sports activity. An accidental collision with another player led to a forceful impact on the upper jaw, causing immediate pain and dislodging of the upper front teeth. The patient sought immediate medical attention following the incident, and the displaced teeth were repositioned.

Subsequent Dental Care:Following the initial emergency treatment, the patient underwent a series of dental procedures to address the aftermath of the trauma. These included splinting of the affected teeth, endodontic therapy, and the placement of temporary crowns. Additionally, periodic follow-up visits were scheduled to monitor the progress of the affected teeth.

Symptoms at Presentation:Upon examination during the current consultation, the patient reported experiencing occasional sensitivity, particularly while consuming cold or hot beverages. Furthermore, the patient expressed concern about the aesthetic appearance of the affected teeth, describing a slight discoloration and minor irregularities in shape.

Clinical Examination:On clinical evaluation, the previously traumatized teeth, namely tooth #11 and #21, were observed to be intact but displayed signs of minor discoloration and a subtle change in contour. Radiographic assessment revealed no signs of periapical pathology, indicating successful prior endodontic treatment.

Treatment Plan:Given the patient's history of traumatic dental injury and the subsequent treatment received, a comprehensive treatment plan has been devised. This plan encompasses aesthetic restoration through a combination of vital tooth bleaching and minimally invasive composite restorations. Additionally, the patient will be educated on proper oral hygiene practices and advised on the importance of regular follow-up visits for long-term dental health.

Prognosis:Despite the traumatic nature of the injury, the diligent and timely intervention following the incident, coupled with appropriate dental care, has contributed to a favorable prognosis. The patient's commitment to maintaining good oral hygiene will play a crucial role in ensuring the long-term stability and aesthetics of the affected teeth.

Follow-Up:A structured follow-up schedule has been established to monitor the progress of the proposed treatment plan and address any emerging concerns promptly.
