Patient Details
Chief Complaint
Patient reported pain in the upper left back tooth region, especially on chewing and at night.
History
Pain was moderate to severe, persistent for the last few days, and aggravated with hot and cold food.
Medical History
- No relevant medical history.
Clinical Examination
On examination, the upper left maxillary molar (tooth 27) showed:
- Deep caries extending to the pulp.
- Tenderness on percussion.
- No swelling or sinus tract present.
Investigations
- Radiograph (IOPA) Revealed caries involving the pulp.
Diagnosis
- Irreversible Pulpitis in relation to tooth 27.
Treatment Plan
- Root Canal Treatment (RCT) was advised to relieve pain and save the natural tooth.
Treatment Performed
- Local anesthesia administered.
- Caries removed and access opening made.
- Working length determined using apex locator and radiograph.
- Cleaning and shaping done using rotary endodontic instruments.
- Copious irrigation with sodium hypochlorite and saline.
- Intracanal medicament placed and temporary restoration given.
- On the next visit, canals dried and obturation was done with gutta-percha.
- Permanent restoration was placed, followed by advice for a crown.
Outcome
- Patient’s pain subsided completely after the procedure.
- Tooth was restored and function retained.
- Patient advised follow-up and placement of a full-coverage crown for long-term protection.
