BARIATRIC SURGERY -A TREATMENT OF TYPE 2 DIABETES A case report - Surjeet Singh (name changed) operated at Criticare Hospital was 200 kg, BMI-55, at the time of Surgery. He was suffering from hypertension,Type 2 Diabetes, Sleep Apnoea, and Bilateral osteoarthritis of knee joint, Dysponea on walking few steps. He underwent Sleeve Gastrectomy and within few months his diabetes was under control without any medication and other co-morbid symptoms also started improving. Since earliest description several thousand years ago, diabetes has remained a chronic progressive disease. The disease now affects ~2 million people worldwide, diabetes related to expected to increase by > 50% in next 10 years. The prevalence of Diabetes among elderly has increased 63% in 10 years. Diabetes is now ranked the 6th leading cause of death by disease in US. In early 1980’s surgeons realized that many patients with Type 2 diabetes who had undergone Gastric Bypass for the treatment of Morbid Obesity experienced a complete remission. This remission proved durable. In the meta-analysis of 22094 patients Buchwald et.al found diabetes resolution in 98.9% of patients undergoing Biliopancreatric Diversion or duodenal switch, 83.7% resolution of diabetes after Gasric bypass and 47.9% after Lapascopic Gasric Binding. Sleeve gastrectomy has also shown good results~86% in type2 diabetes correction. The benefits of Diabetes resolution accomplished by Surgery are significant. Diabetes related mortality by Roux-en- Y Gastric Bypass have been followed for over a period of 10 years and was found 92% decrease as compared with controls. The economic medical and social burden of diabetes is immense. With our present resources of medical management, Diabetes remission and reduced death rate due to diabetes related disease has not been achieved significantly. So the Bariatric Surgery (Metabolic Surgery) represents a new frontier in diabetic treatment.
