Shampa and Pritam (names changed) walked into our clinic devastated. 

Her AMH was 0.7. 

His sperm motility was low.

They had already been told: “You cannot conceive with your own gametes. Go for donor egg and donor sperm IVF.” 

They almost believed it was over.

But the truth was different.

What does low AMH really mean?

AMH reflects ovarian reserve — the number of eggs, not necessarily their quality. 

An AMH of 0.7 is not zero. 

If ovulation is occurring, pregnancy is still possible — sometimes even with less complex treatment.

Low AMH does not mean the end. 

What does low motility mean?

Reduced motility means sperm move slower. 

But if count is adequate, morphology acceptable, and proper lab processing is done, many couples conceive with IUI. 

Male factor does not mean the end of fatherhood. 

Our approach 

  • No blame. 
  • No one-sided treatment.
  • Infertility is a couple’s condition. 

We chose a stepwise, rational plan:“Let’s try IUI once.” 

She conceived in the first cycle.They became biological parents — to twins. 

The second chapter 

At 28 weeks, one baby showed growth restriction (IUGR). 

This required: 

  • Serial growth scans 
  • Doppler monitoringFetal medicine supervision
  • High-risk pregnancy care
  • Planned delivery at 36 weeks with NICU backup.

Today, mother and both babies are healthy. 

Take-home lessons:

Low AMH is not the end of hope. 

Male factor is not the end of parenthood. 

Donor is not always the first answer. 

Treat the couple — together.

Conception is only half the journey; pregnancy care matters equally.

Medicine is not just about reports.It is about judgment, patience, and teamwork.
