Chief Complaint
The patient complained of pain in the upper left back teeth and food lodgement between teeth 26 and 27.
Clinical Examination
- Deep caries involving the pulp in both 26 and 27
- Gingival recession noted interdentally, forming a visible gap
- Tender on percussion, with food impaction
- No tooth mobility
Radiographic Findings
- Caries approaching the pulp in both 26 and 27
- No periapical pathology
- Mild PDL widening observed
Diagnosis
- Irreversible pulpitis in 26 and 27
- Open gingival embrasure due to localised recession
Treatment Plan
- Root Canal Treatment for both teeth
- Fabrication of a joint zirconia crown covering 26 and 27
- Incorporation of a custom tag in the crown to close the recession gap
Procedure
- RCT performed using standard protocol
- Tooth prep done for full coverage crowns
- Joint zirconia cap planned for improved strength and contour
- Lab instructed to add a gingival tag in the interproximal area
- Crown trialled for fit, occlusion, and contact
- Final cementation
Outcome
- Pain resolved
- Food lodgement eliminated due to effective embrasure closure
- Good aesthetics and functional restoration achieved
Follow-Up
- At the 2-week and 1-month reviews, the patient was comfortable and satisfied
- The crown remained intact with no complications
