Chief Complaint

The patient complained of pain in the upper left back teeth and food lodgement between teeth 26 and 27.

Clinical Examination

  • Deep caries involving the pulp in both 26 and 27
  • Gingival recession noted interdentally, forming a visible gap
  • Tender on percussion, with food impaction
  • No tooth mobility

Radiographic Findings

  • Caries approaching the pulp in both 26 and 27
  • No periapical pathology
  • Mild PDL widening observed

Diagnosis

  • Irreversible pulpitis in 26 and 27
  • Open gingival embrasure due to localised recession

Treatment Plan

  • Root Canal Treatment for both teeth
  • Fabrication of a joint zirconia crown covering 26 and 27
  • Incorporation of a custom tag in the crown to close the recession gap

Procedure

  • RCT performed using standard protocol
  • Tooth prep done for full coverage crowns
  • Joint zirconia cap planned for improved strength and contour
  • Lab instructed to add a gingival tag in the interproximal area
  • Crown trialled for fit, occlusion, and contact
  • Final cementation

Outcome

  • Pain resolved
  • Food lodgement eliminated due to effective embrasure closure
  • Good aesthetics and functional restoration achieved

Follow-Up

  • At the 2-week and 1-month reviews, the patient was comfortable and satisfied
  • The crown remained intact with no complications
