  • Background and Medical History: The patient, a 72-year-old female, presented at Summirow Dental with a primary complaint of tooth cavities, bad odour, and significant deterioration in the appearance of her teeth. She had been referred by her primary care doctor, who was concerned about her deteriorating oral health, its possible effects on her nutrition, and its potential connection to her overall well-being.

In addition to her dental issues, the patient was also suffering from a compromised lung condition, which was exacerbating her ability to eat and maintain proper nutrition. The doctor inquired whether improving her oral health would lead to improvements in her overall health. Upon initial examination, it was clear that the patient’s oral health was significantly compromised. Her gums were inflamed, and there was severe periodontitis (pyrrohea). Her ability to chew was compromised, further complicating her nutrition.

This had a direct impact on her general health, with consequences that extended beyond oral discomfort.

Diagnosis:

Oral Health Diagnosis: Severe tooth mobility, widespread tooth decay, and significant gum and bone loss.

Systemic Considerations: The patient's poor nutrition and compromised lung function, were potentially worsened by her inability to properly chew and eat.

Treatment Plan and Discussion:

After a comprehensive examination, including diagnostic scans and an assessment of the patient’s overall health, we explained to her that her oral health and general health were interrelated. We informed her that improving her dental health would likely contribute to an improvement in her overall health. This would, in turn, enhance her ability to eat, digest, and maintain better nutritional levels.

Given the advanced nature of her periodontitis, we explained that her teeth could not be salvaged and would need to be extracted.

We presented two options for tooth replacement:

1. Removable Dentures: A removable prosthesis, which could be taken out for cleaning and maintenance.

2. Dental Implants: A fixed option involving the placement of implants that would serve as a permanent replacement for the extracted teeth.

The patient expressed a preference for dental implants, as she was not comfortable with the idea of a removable prosthesis, which she had previously feared. She was concerned about the aesthetics and functionality of removable dentures.

Treatment Execution:

Day 1 – Tooth Extraction and Implant Placement:

Extraction of All Remaining Teeth: We performed a thorough extraction of all the patient's remaining teeth due to their poor condition and advanced decay.

Placement of Dental Implants:

After extractions, we immediately placed dental implants into the jawbone. This was done in a single procedure, ensuring that the patient was not required to undergo multiple surgeries.

Day 2 – Impression for Fixed Denture:

The following day, we took the necessary measurements and impressions to create a fixed denture that would be placed on top of the implants. These impressions were sent to the dental lab for the fabrication of the final prosthesis.

Outcome and Patient Satisfaction:

The patient was visibly relieved and satisfied with the procedure, noting that she was experiencing far less discomfort than she had anticipated. She expressed happiness at not needing a removable denture, which had previously been a source of stress for her.

In the follow-up consultation, she reported a significant improvement in her confidence, appearance, and ability to chew. This improvement in her oral health led to better nutritional intake and overall well-being. Furthermore, the fixed implants provided her with a more stable and functional solution, restoring her quality of life and enhancing her physical and mental health.

Conclusion:

This case highlights the importance of addressing oral health issues, particularly in older patients with significant systemic concerns, such as compromised nutrition and lung conditions. By addressing her advanced periodontal disease and offering dental implants as a solution, we were able to significantly improve the patient's oral health and, consequently, her overall health.

The patient's satisfaction with the results was evident, and she appreciated the fixed solution over the removable prosthesis. This case demonstrates how a holistic approach to dental care—one that considers both oral health and systemic health—can greatly enhance a patient’s life, especially when they have been delaying treatment due to misconceptions about available options.
