As we get older, our dental needs evolve just like the rest of our health. Today, we’d like to share the story of one of our patients a 57-year-old woman who visited our clinic with a common but frustrating issue: food getting stuck in her upper left back tooth.
Let’s take a closer look at what was causing the problem and how we helped restore her comfort and oral health.
Patient Complaint: Food Lodgment in the Upper Left Back Tooth
Our patient reported that food frequently gets trapped between her upper left teeth while eating, especially in the area toward the back of her mouth. This made eating uncomfortable and left her constantly reaching for dental floss or toothpicks.
Such complaints are common and shouldn't be ignored, as they can be early signs of decay, gaps, or faulty fillings.
Clinical Examination: The Culprit Revealed
Upon clinical examination, we identified a Class II cavity in tooth number 26 (that’s the upper left first molar, for those unfamiliar with dental numbering).
A Class II cavity means the decay is located on the side surface of the tooth that touches the neighbouring tooth. These cavities are notorious for causing food lodgment because they affect the contact point between teeth.
Here’s what we observed:
- A visible cavity between teeth 26 and 25.
- Slight gingival inflammation due to food trapping.
- No signs of tooth fracture or pulpal involvement.
- The patient had good general oral hygiene but localized plaque in the affected area.
Diagnosis: Class II Carious Lesion in Tooth 26
This type of cavity usually results from food and plaque accumulating between teeth, especially if flossing isn't regular or if fillings have lost their shape or tight contact.
Fortunately, the decay hadn’t reached the pulp (nerve) of the tooth, so we had a conservative and effective solution.
Treatment Provided: Composite Restoration
We recommended a Class II composite restoration to treat the decay and restore proper contact between the teeth.
Here’s how the procedure went:
- Local Anesthesia: We numbed the area to ensure the patient’s comfort throughout the treatment.
- Decay Removal: All decayed tooth structure was carefully removed, ensuring no bacteria or weakened enamel was left behind.
- Matrix Band Placement: A matrix band was used to recreate the natural wall of the tooth and help shape the contact point.
- Etching and Bonding: The prepared cavity was etched, rinsed, and dried. Then we applied a bonding agent to create a strong bond between the tooth and the filling.
- Composite Filling: A tooth-coloured composite resin was layered into the cavity and cured (hardened) using a special light.
- Finishing and Polishing: The filling was trimmed, shaped, and polished to match the natural contours of the tooth.
Outcome: Immediate Relief and Long-Term Benefits
The patient experienced immediate relief from food lodgment. With the contact point restored, food no longer gets trapped in that area, and her gums have already started healing due to the improved oral environment.
We advised her to maintain the area with regular brushing, flossing, and periodic dental check-ups to prevent future decay.
Patient Education: How to Prevent Food Lodgment and Cavities
Here are a few helpful tips we shared with our patient and now with you:
- Floss Daily: Especially in hard-to-reach back teeth, flossing prevents debris buildup.
- Use Interdental Brushes if gaps are wider or flossing is difficult.
- Routine Dental Visits: Every 6 months helps detect early signs of decay.
- Watch Sugar Intake: Especially sticky or sugary foods that cling to molars.
- Use Fluoride Toothpaste: Strengthens enamel and helps prevent cavities.
