Cervical Cerclage: Why and How?

Why is Cervical Cerclage Recommended?A cervical cerclage is often recommended for women who have an incompetent cervix (also known as cervical insufficiency).
This condition can cause the cervix to open prematurely, increasing the risk of miscarriage or preterm birth. The goal of the cerclage is to reinforce the cervix during pregnancy and help it stay closed until the pregnancy reaches a safe point.ACOG Guidelines
:According to the American College of Obstetricians and Gynecologists (ACOG), cerclage is recommended when:There's a history of second-trimester pregnancy losses due to painless cervical dilation.
Ultrasound reveals a shortened cervix before 24 weeks of gestation in a current pregnancy.Certain high-risk cases with multiple pregnancies.How is it Performed?
Cervical cerclage is usually done through a transvaginal procedure:A stitch (suture) is placed around the cervix to reinforce it.It is generally performed between 12 and 24 weeks of pregnancy.
The suture is often removed around 36-38 weeks or earlier if needed.Additional Precautions:Post-cerclage, extra precautions might be needed:Bed rest or reduced activity may be recommended, depending on your condition.
Avoid heavy lifting and strenuous activities.Regular follow-up with your OB-GYN for monitoring of cervical length and overall health of the pregnancy.
In some cases, progesterone therapy may also be used alongside the cerclage to further reduce the risk of preterm birth.Understanding cervical cerclage can reduce anxiety and help ensure that you are taking every precaution for a healthy pregnancy journey.
