ANTERIOR COMPARTMENT SYNDROME occurs due to raised pressure within the anterior compartment of the lower leg (front of theterbia). A rise in pressure is caused due to bleeding, swelling in tibialis anterior muscle resulting from: Direct trauma (causing swelling within the compartment), Fracture, Soft tissue injury, A rapid increase in size & volume of the muscle within the compartment, Vigorous exercise, Burn or Gradual tightening of the surrounding connective tissue. SYMPTOMS of Anterior compartment syndrome are: Aching, tight, cramping, or squeezing pain (shin pain). If the swelling causes the compression of the nerve you may also experience paraesthesia & weakness (you will feel difficulty in elevating your foot). There may be foot drop seen in severe cases. DIAGNOSIS can be made with a thorough history, physical examination, compartment pressure testing & bone scanning. PHYSIOTHERAPY TREATMENT of Anterior compartment syndrome: stop the activity that aggravates the symptoms, Apply ICE, Taping (anterior compartment syndrome taping can provide relief from painby supporting the muscle), Keeping the body part below the level of the heart(it improve blood flow), Orthotic inserts (it will correct the biomechanics oflower limb and reduce the excess load from the tibialis anterior muscle), Soft tissue massage, Postural realignment, Hydrotherapy, Stretching exercise, Static cycling (for cardiovascular training without exaggerating the symptoms), Iso-kinetic muscle strengthening exercise. Some severe cases require surgical management to correct the condition. Common surgical treatment for anterior compartment syndrome is Fasciotomy, in which the surgeon cutting the fascia and open it torelieve the pressure.
