Hello It was an older concept wherein the doctor would suggest you to come for the tooth removal once the pain had subsided after taking medication, but nowadays the source of infection that is the wisdom tooth has to be removed irrespective of it is paining or not because only after it's removal you are going to get relief from the pain and yes this would be done by an oral and maxillofacial surgeon who has expertise in this field. This is done under local anesthesia which is an injectable form of anesthesia. The spray anesthesia that you're talking about would be something that would lead to the whole body anesthesia that is general anesthesia that is not at all required for this procedure. It's a very normal routine dental procedure that you can get done

Wisdom tooth should be removed if recommended by the dentist. The dentist will give you anaesthesia to numb the area and hence you feel not feel the pain which you are currently facing. The only time dentist might ask you to take antibiotics before removing the tooth is when you can’t open your mouth fully or have a big swelling or fever. Else you can get your tooth removed even if you have pain. Medication is followed after that helps further decrease the pain and fasten healing.
