Hello As you described and what pictures say- the condition can be due to impacted third molar teeth (upper and lower both). Which might have aggravated due to smoking. You need to consult a professional for the accurate diagnosis. If you want we are available over a video consultation through this . And we are in NCR area itself so physical consultation will not be a issue. Thanks
Next Steps
Do get a consultation done
Health Tips
Do warm saline rinses

It can be leukoplakia or due to chronic cheek biting. Antioxidants along with multivitamin supplements has to be taken. Impacted wisdom tooth has to be removed. You can consult through this for prescription of medication to relieve the lesion.
