Ondem (Ondansetron) is a safe and commonly used anti vomiting medicine in children when used in the correct dose. ✅ Advices You can give Ondem syrup if available. The usual dose is 0.15 mg per kg body weight, which means approximately 2 ml of Ondem syrup (4 mg per 5 ml) for a child weighing around 15 kg. Give the medicine only once, and wait for 20 to 30 minutes before giving small sips of ORS or water. Keep the child upright and avoid giving food until vomiting settles for at least one hour. What to Monitor Frequency of vomiting and ability to retain fluids. Urine output and alertness of the child. When to Consult a Doctor Immediately If vomiting continues even after taking Ondem. Syrup ONDEM(2mg/ 5 mL ) 5 mL can be given safely to the child at least 40 min about the food , thrice daily for 1 day followed by sos And kindly provide complete details regarding weight of child , whether he is having fever / abdominal pain / loose stools / blood in stools.

First — what matters most From your note: 4½-year-old, vomited 4–5 times, you’re 2–3 hours from town. That can be a self-limited viral gastroenteritis, but it can also progress to dehydration or be something that needs urgent care. So we manage by watching for danger signs and starting simple supportive measures immediately. Do this straight away (no medicines): 1. Check for dehydration now: look for dry mouth, few or no tears when crying, sunken eyes/fontanelle, decreased urine (no wet diaper/pee for ≥6–8 hours), lethargy/very sleepy, or fast breathing. If any of these are present — go to the nearest health centre or ER now. 2. Give tiny frequent sips only: offer ORS (oral rehydration solution) or clean water in very small amounts — spoon or syringe: ~5–10 ml every 1–2 minutes. If she tolerates that, increase gradually (target ~50–75 ml every 10–15 minutes as tolerated). Don’t force large volumes at once. 3. Avoid solid heavy food for now; small bland sips/ice chips only. If breastfeeding, continue breastfeeding in small frequent feeds. 4. Avoid antiemetics or other medicines unless seen by a doctor — they can mask important symptoms and dosing depends on weight. 5. Keep her comfortable: rest, keep cool, offer plain fluids. If there’s fever you can treat later but don’t give any medicine without confirming weight and allergies. 6. Note anything abnormal: blood in vomit, green bilious vomit, severe abdominal pain, high fever, breathing difficulty, convulsions, confusion, or if vomiting becomes projectile — these require immediate transfer to hospital. When to not wait 2–3 hours and go immediately: • Signs of moderate-to-severe dehydration (listed above). • Child becomes drowsy, floppy, not responding, has rapid breathing, or has bloody/green vomit. If none of the above are present and the child is alert and taking small sips, you can continue the sip-and-watch plan and head to town — but keep monitoring every 15–30 minutes. About ondansetron (ondem) — short, frank: Ondansetron can help vomiting in children but should only be given after a doctor assesses the child’s condition and weight. Don’t give it blindly in a remote setting — it can hide red flags and dosing errors are risky.
