Vitamin B12 deficiency is treated based on the severity of the deficiency and the underlying cause (e.g., poor diet, malabsorption, or conditions like pernicious anemia). Here's a complete overview:
1. Treatment Options
A. Oral Vitamin B12 Supplements
Form: Cyanocobalamin or methylcobalamin
Dose:
Mild to moderate deficiency: 1000–2000 mcg daily
Maintenance (after correction): 500–1000 mcg daily
Effective if: The gut can absorb B12 (i.e., not due to malabsorption)
B. Injectable Vitamin B12
Form: Usually cyanocobalamin
Dose (standard regimen):
1000 mcg intramuscularly (IM):
Daily or every other day for 1–2 weeks (until symptoms improve)
Then weekly for 1 month
Then monthly for maintenance, often lifelong if cause is permanent (e.g., pernicious anemia)
C. Sublingual (under the tongue) B12
As effective as oral in many cases
Dose: 1000–2000 mcg daily
2. Identify and Treat the Cause
Dietary (e.g., vegan/vegetarian): Add B12-rich foods or continue oral supplements
Pernicious anemia: Lifelong IM or high-dose oral therapy
Gastrointestinal issues (e.g., Crohn’s, gastric surgery, metformin use): May need injections
3. Dietary Sources of Vitamin B12
Animal products only:
Liver, meat, fish, eggs, dairy
Fortified foods: Cereals, soy milk (check label)
4. Monitoring
Recheck B12 levels after 4–8 weeks of treatment
Monitor symptoms (fatigue, neuropathy, memory, etc.)
Long-term: Check every 6–12 months if on maintenance
5. Symptoms of B12 Deficiency
Fatigue, weakness
Tingling or numbness in hands/feet
Memory issues, mood changes
Pale skin or glossitis
Anemia (macrocytic)
If untreated: irreversible nerve damage
If you'd like, I can suggest a treatment plan tailored to your current B12 level, symptoms, or medical history.
Take tab neurobion one morning and one evening for 6 to 8 weeks.
Take sprouted grains, yeast fermented foods, curd, cheese, fortified plant based milks, eggs.
Dear patient,
Your Vitamin B12 level of 178 pg/mL is indeed low (normal range: 200–900 pg/mL), which can explain your symptoms of weakness, body pain, and low energy. Even though your hemoglobin is normal, B12 deficiency can still affect your nervous system and overall vitality.
Next Steps
1. Vitamin B12 Supplementation
• Option 1: Oral tablets (e.g., Methylcobalamin 1500 mcg daily for 2–3 months).
• Option 2: If deficiency is severe or symptoms worsen – Intramuscular B12 injections (1000 mcg once a week × 4 weeks, then monthly).
2. Dietary Tips
• Include B12-rich foods like milk, curd, paneer, eggs, chicken, and fish (if non-vegetarian).
• Vegans may need long-term supplements.
3. Recheck Levels
• Re-test your Vitamin B12 after 6–8 weeks of treatment to assess improvement.
4. Lifestyle
• Get adequate sleep, hydration, and physical activity to help restore energy levels.
