I am suffering from Silent Reflux (LPR) which affects my vitamins. Currently my Vitamin B12 level 277.9. Pls suggest supplement and whether to consultant with General Physician, endocrinologist, nutritionist, gastroenterologist. Who can advise best for Vitamin deficiency.

Your vit b12 level is not that much low that you have to worry for.
If you are a vegetarian than you can take
Tab vit b complex - 1 tab - once daily
But if you are a nov vegetarian you can eat animal products which is a great source of vit b12.
When taking medication must drink plenty of water.

Vitamin B12 level of 277.9 may be on the lower side of the normal/borderline range depending on the laboratory reference range. However, the B12 value alone is not sufficient to decide whether you need supplementation or injections.
Silent reflux (LPR) itself does not necessarily mean that you have Vitamin B12 deficiency. However, certain factors such as prolonged use of acid-suppressing medicines (for example, PPIs), dietary intake, absorption problems and other gastrointestinal conditions can contribute to low B12 levels.
The appropriate treatment depends on your symptoms, diet, medications, duration of reflux, previous B12 levels and other blood investigations. Depending on the situation, oral B12 supplementation may be sufficient, while injections are required only in selected cases.

Lower level of normal is not a problem unless you develop symptoms secondary to it. Like tingling sensation in your limbs, fatigue, weakness. But lower than that could lead to serious neurological effects. Increase your non vegetarian diet, green leafy vegetables if still it doesn't help then switch to meds. Worrying won't help. Your Vitamin B12 levels fall in the normal range if the unit is in pmol/litre.Silent reflux in itself is highly unlikely to cause vitamin B12 deficiency.

Your B12 level of 277.9 is not clearly deficient; please confirm the units and laboratory reference range. LPR does not usually cause B12 deficiency. Avoid high-dose supplements without checking your CBC, folate and, if needed, methylmalonic acid. If you have numbness, weakness or anemia, arrange a clinical review promptly.

if your vit b12 is low but if you are vegetarian then you need to supply via injection of vitb12 course of 5 injections and also you have to eat curd, green leafy vegetables and also soya chunks but if you are prefer to egg then you have to eat every 4 egg eat daily then your all vitamins will be maintain also eat daily fruits vegetables don’t worry if you follow all this then all vitamins will be stable and maintain thank you be happy have a nice day☺️
You should consult a Gastroenterologist and then a nutritionist if your daily life is getting affected. Otherwise just consume a balanced diet, fruits, fluids and do physical activity.

Start with a general physician for your B12. They can look into the cause and plan replacement for you, while keeping your blood pressure treatment in view.
