Your culture sensitivity report shows Resistance to Ciprofloxacin, Nalidixic acid, Ampicillin, Amoxycillin and even Nitrofurantoin titre is 32 which is virtually ineffective. You should have been given injectable Gentamycin or Amikacin to which the bacteria are reported to be sensitive.

There can be many reasons 1) The dose which you were given was inadequate 2) course was short 3) infection is resistant to this antibiotic 4) get blood sugar tested 5) infection is high up in kidney, so oral medicine not helping

You have to get repeat urine RE/ME and culture done. With the present report I would have preferred to use Ceftriaxone because the spectrum of activity of Ceftriaxone is much wider than that of Nitrofurantoin Anyway now you should wait for the next culture report.

The MIC for Nitrofurantoin is very high.. very unlikely to have effect.. ideally if you have severe infection.. Repeat a urine examination.. if it is still showing pus cells.. may need injectables.. Can take Flovxate 200mg Bd for 5 days meanwhile..
