Your symptoms suggest that you might be having adenomyosis which can be treated with Mirena IUS which is the preferred treatment option or else you may need a hysterectomy (uterus removal surgery). This is called Adenomyosis and it happens after 40 yrs and it can be treated with Mirena and uterus removal both depending upon your wish
