Hi doc, i got scratched from some rusted iron stuck on my soap, so i am confused if i should take tt vaccine. I have taken tt last on March 21 this year.
Some say single dose valid for 3 month some says 6. Not sure what to do.
My history of vaccine shot:

I am not sure whether I completed the primary TT vaccination course in childhood.

I am Dr. Shamoon. You actually have a very reassuring vaccination record, so let me settle the confusion. Your three doses - 11 June 2025, 28 November 2025 and 21 March 2026 - are spaced exactly like a proper primary tetanus series (0, about 5-6 months, and about 4 months later), so even if you were unsure about your childhood immunisation, you can consider yourself fully primed now. After a completed primary course, protection lasts around 10 years, and for a dirty or rusty wound the accepted rule is a booster only if the last dose was more than 5 years ago. Your last dose was barely a few months ago, so you do NOT need another TT or Td shot for this scratch, and repeating it too soon only increases the chance of a painful, swollen arm (an Arthus-type reaction) without adding protection. The "single dose valid for 3 months / 6 months / 1 year" advice you have heard is a common misconception - that short window applies to tetanus immunoglobulin (TIG/ATS antibody), not to the vaccine. Please also remember that rust itself does not cause tetanus; the risk comes from soil and dirt contamination, so what matters most for you now is proper wound care rather than another injection.

1) No tetanus injection is needed for this scratch given your 21 March 2026 dose - skip it unless a doctor examining the wound finds it deep, punctured or grossly contaminated with soil. 2) Wash the scratch under running water with plain soap for 5 minutes, remove any visible dirt or rust particles, pat dry and apply an antiseptic such as povidone-iodine. 3) Keep it covered with a clean dry dressing for 2-3 days and change it daily or whenever it gets wet. 4) Do not apply turmeric, toothpaste, oil or ash, and do not squeeze or scratch it. 5) Watch the wound for 5-7 days for spreading redness, swelling, throbbing pain, pus or fever - these mean bacterial infection (needing antibiotics), which is far commoner than tetanus. 6) You may take plain paracetamol 500 mg if it is sore. 7) Note your next tetanus booster is due around March 2036, or 5 years from now if you get another dirty wound - save the 21 March 2026 date in your phone so you never have to guess again. 8) If you are still unsure or the wound looks deeper than a scratch, send me a photo on a Practo consult or come to Synergy Multispeciality Hospital, Miraj, and I will examine and advise.

You don't need vaccine.
Your last shot of TT was this year 21st March. Antibodies against clostridium tetani will be running in your blood. So don't think much.

Since your last tetanus-containing vaccine was on 21 March 2026, you do not need another booster now provided you have previously completed the primary tetanus vaccination series. Please clean the scratch thoroughly with soap and running water. If your childhood vaccination history is genuinely unknown/incomplete, consult a doctor for appropriate tetanus prophylaxis.
