Hello,
This problem is very common nowadays.
Just start working on your pectoral muscles(chest muscles) and shoulder muscles.
Your trapezius muscle may get overused due to tightning of surrounding muscles ,so start working on surrounding muscles and joints.
Since you have taken Physiotherapy and your ultra sound shows thickened muscles. You should go for MRI to rule out the root cause of your pain and for detailed information.

Next Steps
Firstly you'll need to modify your posture if its incorrect.
Secondly you'll benefit from Manual therapy and some continuous exercises
of adjacent joints and muscles.
This is needed to support the nearby structures.
Health Tips
Avoid prolonged postures of flexed neck and upper back.
Isometric exercise with PNF inhibition and hold relax technique will help you out. Being an Occupational Therapist i would suggest you to follow those exercises. Manual occupational therapy will be helpful. You may also check video on
