Hi this is her lower left back tooth which is the first molar.
Keeping in mind this is her permanent tooth and from the picture the cavity or the extent of decay looks too deep, this tooth requires treatment in order to save it.
Toothache is because the decay has progressed to the inner most layer of the tooth called the pulp, infecting it.
A root canal treatment would be ideal as the child needs the tooth for chewing further on.
