Hello! Wisdom tooth extraction is required as there is cavity and you also have pain. As it dsnt play any role in function and looks, it's better to get it removed.

Hello. Is the pain localised to one particular tooth/ area? The second picture of your OPG suggests that you have a deep cavity with respect to your lower right wisdom tooth.

Hi this is Dr.Harsha, if the pain is in the wisdom tooth area where it's highlighted in second image, it should go for extraction, but if it's generalized like everywhere in mouth, then should evaluate the condition clinically
