It is always by titration against your TSH level. There is no formula to get the dose of Eltroxin.

Your T3 and T4 levels are normal and TSH level is  elevated.If it was the similar report before you started elthroxin 50 ,it was subclinical hypothyroidism where TSH level is high and T3 and T4 levels are normal.In such instances treatment is given only if you are symptomatic ,or your TSH level is above 10 .
