If you have an impacted third molar (wisdom tooth) that is causing significant pain and discomfort, it's a good idea to consult with a dentist or oral surgeon for further evaluation and recommendations. Impacted wisdom teeth can cause various issues, including pain, infection, and damage to adjacent teeth. Given that your wisdom tooth is mesioangular and close to nerves, extraction might be considered, especially if it's causing persistent pain that is not responding to painkillers. However, the decision to extract the tooth should be made in consultation with a dental professional who can assess your specific situation, review the OPG, and take into account factors such as the tooth's position, your overall oral health, and potential risks associated with extraction. Before making any decisions, it's important to have a thorough discussion with your dentist or oral surgeon about the possible treatment options, the risks and benefits of extraction, and any other considerations. They can provide personalized advice based on your dental history, clinical examination, and imaging results. If you're concerned about the pain and discomfort you're experiencing, seeking professional guidance is the best course of action.

Definitely opt for surgical extraction of the teeth. The pain will be recurring if not done.

Better to visit a local oral and Maxillofacial surgeon for physical evaluation and treatment. Most probably extraction/removal of tooth is required.

You should get it extracted if its causing you discomfort. You can also share your X ray here. In some cases it can also affect the adjacent tooth so it’s advisable to get it extracted.
