Lower abdominal pain with a sensation of pressure or tenesmus (feeling to pass stool even when bowels are empty) often points towards Pelvic Congestion, IBS (Irritable Bowel Syndrome), or a mild Pelvic Inflammatory Disease (PID), especially if it hurts on pressing. Since you are not constipated and already taking probiotics, the issue might be related to intestinal spasms or inflammation. I recommend getting a USG (Abdomen and Pelvis) done to rule out any ovarian cysts or internal inflammation. For immediate relief, an anti-spasmodic might be more effective than just antacids."

Health Tips
Try a low-FODMAP diet for a few days—avoid gas-producing foods like cauliflower, cabbage, beans, and heavy dairy. Instead of a hot water bag, try light pelvic stretching exercises. Keep a 'Pain Diary' to see if the pain is related to your menstrual cycle, as that provides a big clue for diagnosis."
If you experience any sudden sharp pain, fever, or spotting/bleeding between periods, please consult a gynecologist immediately. Avoid self-medicating with strong painkillers as they can mask the actual cause of the pain or irritate your stomach lining further."
