But the thread changing sessions went on for 2 months & even after that there was lot of pain & didn't seem like it was healing. So I underwent another fissure tissue removal surgery 4 months ago. I had followed everything doc had told. I was getting better. But I have had pain & can't manage a day without 2 sitz bath. Moreover even after following all that, I've been having a strange lower abdominal pain & further the whole lower body starts paining. More like pulling & burning .I even have the anal pain. There's no bleeding or pus. But pain.I really have no energy to undergo another surgery.This is my 5th surgery & I have other health probs too. Already lost 20kgs coz of previous surgeries.Feeling more weak now. Also want to mention that in college, i had eaten lot of slate pencils for 3 years.Is that the reason?Now i don't eat. I can't quit my job too.Please help & guide 😭 painful

Going through five procedures by the age of 21 is exhausting, and I want to reassure you that your present problems need investigation, not necessarily a sixth operation. The continuing anal pain 4 months after fissure surgery, needing two sitz baths a day, usually comes from anal sphincter spasm with a non-healed or recurrent fissure, or from scarring/narrowing after repeated kshara sutra and surgery - most of this is managed medically with sphincter-relaxing ointment, stool softeners and pelvic floor physiotherapy. The newer pulling, burning lower abdominal pain going down both legs with the whole lower body aching is a separate issue and is not explained by the fissure: in a young woman it points to pelvic floor myofascial pain, a gynaecological cause such as an ovarian cyst or endometriosis, a urinary cause, or nutritional deficiency and anaemia from your 20 kg weight loss and poor eating. Importantly, eating slate pencils for 3 years is pica, a classic marker of iron deficiency - that alone can cause aching legs, breathlessness, weakness and much poorer pain tolerance. So yes, your college habit may well be a clue, but to the deficiency, not to the pain directly.

1) Blood and imaging first: complete blood count with peripheral smear, serum ferritin and iron studies, vitamin B12, vitamin D, thyroid profile, blood sugar, ESR/CRP, urine routine, and ultrasound of abdomen and pelvis. Ferritin and the pelvic scan are the key two for you. 2) One thorough examination by a colorectal surgeon - if the area is too tender, an examination under anaesthesia - to record whether there is a residual fissure, a stricture or only spasm. Ask specifically about non-surgical options before consenting to any further procedure. 3) A gynaecology opinion for the lower abdominal pain radiating to both legs. 4) Keep up the sitz baths, high fibre, about 3 litres of water and a stool softener at night so that passing stool is never hard. 5) Start eating in small frequent meals with protein and iron-rich foods - the weakness will not lift while your intake is this low. Supplements should be chosen after the reports rather than guessed. 6) Please carry all your earlier operation notes and reports.
