Steroidal nasal spray
I have been using a steroidal nasal spray for 2+ years. I get severe congestion if I stop using it. Will this periodic usage affect my heart health ? How to slowly taper off the usage

You should get a CT PNS and nasal endoscopy done to know the exact cause of your obstruction.  Such a long usage of steroid spray is not advisable.
