Melatonin is one of the safer sleep aids and, for short-term use in a healthy 21-year-old, it is reasonable â but I want to be honest with you about what it does and does not do. Melatonin is not a sedative; it is a body-clock signal. It works best when the problem is a shifted sleep schedule (going to bed very late, irregular hours, night study/shifts, jet lag) â which is exactly the pattern a hectic schedule creates. It works poorly if the real problem is racing thoughts, stress, caffeine, or screens at night, and in that case a tablet will disappoint you. In India melatonin is available over the counter as 3 mg and 5 mg tablets, but the evidence actually favours a LOW dose â 0.5 mg to 3 mg â taken about 1-2 hours BEFORE your intended bedtime, not at bedtime. Higher doses are not more effective and cause more morning grogginess and vivid dreams. It should be a 2-4 week bridge while you fix the schedule, not a nightly habit.
Next Steps
1) If you want to try it: start with the lowest strength available (0.5-3 mg), taken 1-2 hours before your target bedtime, for 2-4 weeks only, and then stop and reassess. Prefer a reputable, licensed brand â quality varies a lot with over-the-counter melatonin â and avoid combination products that add antihistamines or herbs. I would still prefer you take it after one consultation rather than picking a strip off a shelf, since dose timing is what decides whether it works for you. 2) Pair it with the things that actually reset the clock, or it will not hold: fixed wake-up time every single day including weekends, bright light/daylight within 30 minutes of waking, no caffeine after 4 pm, no screens 1 hour before bed (screen light directly suppresses your own melatonin), and use the bed only for sleep. 3) If you cannot fall asleep in 20 minutes, get out of bed and do something dull in dim light rather than lying there. 4) If sleep is still poor after 4 weeks, the right treatment is CBT-I (cognitive behavioural therapy for insomnia), which outperforms any tablet long term. 5) Also worth checking once: TSH, haemoglobin/ferritin and vitamin D, since deficiency and thyroid issues commonly present as poor sleep in young women. 6) Happy to review this properly with you on a Practo consult and suggest a specific brand and dose for your schedule, or you can see me at Synergy Multispeciality Hospital, Miraj, if you are nearby.
Health Tips
Word of caution: avoid melatonin if you are pregnant or breastfeeding, have epilepsy or an autoimmune condition, or are on blood thinners, fluvoxamine, oral contraceptives or immunosuppressants â these interact. Never combine it with alcohol or sleeping tablets, and do not drive or ride within a few hours of a dose until you know how it affects you. See a doctor rather than continuing tablets if you snore heavily or wake gasping, if you feel low or hopeless, or if the sleep problem persists beyond a month.
