Stop all medicines over the counter as well as herbals for two weeks.
Keep a precise log of all ingestions 24–48 hours prior to flare-ups, including non-prescription analgesics, herbal teas, health supplements, home remedies, and alcohol intake.
Take clear, well-lit close-up images during the initial acute erythematous/pruritic phase before it transitions into a dark patch, as the initial morphology provides critical diagnostic clues.
It's very hard for doctors to diagnose without image of your dark patches. You have consulted so many doctors and done many tests. It would have been nicer if you had posted them along with your question. These things helps doctors to come to a diagnosis easily, especially in skin diseases.
Consult a dermatologist in person for proper diagnosis and management

As the patch is itchy it can be due to any fungal infection but proper evaluation can be done with the picture
Thank you for sharing your concern.
I recommend that you undergo an allergy test to identify the trigger responsible for your allergic reaction.
As you are currently experiencing an active allergic episode, you may take Tab. Fexofenadine 120 mg once or twice daily, as advised by your treating physician.
Once the trigger factor is identified, the most important step is to avoid exposure to that allergen, as this can help prevent future allergic reactions. If the symptoms persist, worsen, or recur frequently despite avoiding the trigger, please consult a physician or an allergist for further evaluation and management.
Proper evaluation and personalised advice are needed for your concern. A detailed consultation would be best to provide accurate guidance.
