Diabetic dermopathy and acquired ichthyosis. Apply moisturizer over hands. Need further history for skin discoloration of leg just to rule out cellulitis, if no inflammatory features can be reassuring.

From the image: • Darkening of skin over the hand • Dry, thickened, rough texture • Prominent skin lines • Hyperpigmentation over knuckles In a 58-year-old diabetic patient, most likely causes are:
Next Steps
Recommended Tests: ✔ HbA1cFasting blood sugarVitamin B12Thyroid profile ✔ Liver function test If pigmentation is also present on: • Neck • Underarms • Groin Then insulin resistance evaluation is important. ⸻ Treatment Approach: ✔ Strict sugar control ✔ Moisturizer twice daily (urea-based or lactic acid-based cream) ✔ Mild soap only ✔ Vitamin B-complex if deficiency confirmed If skin is very thick/dark: • Dermatology evaluation may be needed. No strong steroid creams unless prescribed.
Health Tips
✔ Apply moisturizer immediately after bathing ✔ Avoid harsh soaps ✔ Drink adequate water ✔ Maintain HbA1c below 7% ✔ Use sunscreen on exposed areas This appears to be chronic metabolic skin change rather than something dangerous — but in diabetics, proper evaluation is important to prevent progression. I recommend booking an online consultation so we can review his blood sugar control, lab values, and design a targeted treatment plan rather than just applying creams without addressing the root cause.
