I have severe anal itching that has been going on for more than two years. After pregnancy, I developed piles and took both Ayurvedic and allopathic medicines, and the piles are now mostly resolved. However, I sometimes still have pain while passing stool, which feels like a pin-prick, and occasionally mild bleeding. The itching is very severe both during the day and especially at night, making me extremely uncomfortable. I’m not sure whether it could be pinworms, poor hygiene, piles, a fissure, or another issue. Please guide me on the possible cause and what I should do.

Hi
If you currently don't have piles , Could be worm infestation.( Most likely). Good thing is it's treatable
There is a chance of internal pile which could be causing occasional bleeding.
Needs further details to come to a conclusion
Next Steps
feel free to consult me on practo for further evaluation and management
Chronic itching around Anus can be any of the causes like fissure with residual skin tag, fungal infection, piles which need complete clinical evaluation by a general surgeon or gastroenterologist.
Next Steps
1. Sitz bath- Sit in the tub of plain Luke warm water for 10-15 min, 2-3 times daily
2. Improve dietary fibres- fruits, vegetables, nuts
3. Schedule a clinical examination.
Health Tips
Avoid straining while passing stools, drink plenty of water
The pin-prick pain while passing stool with occasional bleeding points to a chronic anal fissure, and the itching is from moisture and irritation around it - pinworms usually itch only at night and would not give that pain. Two years is long enough that it will not settle with more Ayurvedic or over-the-counter creams. What actually works: keep stools soft (isabgol at night, 3 litres water, plenty of fibre), sitz bath in warm water for 10 minutes twice a day, and a local anaesthetic-nitrate/diltiazem ointment for 6-8 weeks. Wash and pat dry, never scratch or use soap inside.
Next Steps
You need one proper local examination to confirm fissure versus piles. Consult me on Practo and I will start the ointment and stool-softener plan; if it does not heal in 8 weeks, a surgical opinion is the next step.
Consult a general surgeon/proctologist or dermatologist for local examination; anoscopy may be required if bleeding persists.
* If pinworm there then albendazole 400 mg daily for three days can be taken.
* Keep the area clean and dry; wash gently with water and avoid soaps, antiseptics, wet wipes and vigorous scrubbing.
* Wear loose cotton underwear and change it daily.
* Avoid scratching; keep fingernails short.
* Maintain soft stools with adequate fluids and fibre,
* Avoid using steroid-containing or combination “pile/fungal” creams for prolonged periods without medical advice.
