Hi dear, don't be worried, and plz let me knoe if your MRI has been done or not,  if it is a clinical diagnosis or wot?? If your MRI says it's grade 3 tear, then don't hesitate to get operated by a experienced orthopaedic surgeon.  Its a pretty technical surgery but results are gud with gud rehab protocol.
Next Steps
get an MRI done if not done
Health Tips
till the time your are not planning surgery,  keep visiting a expert physiotherapists,  else your brother joint might go into flexion deformity due to weakness of quadriceps muscles

With recent advances in ACL surgeries, you need not worry about the success as almost all surgeries have a very good result. Pre-surgical physiotherapy consultation and follow up for physiotherapy supervised exercises is a must after surgery to gain maximum benifits from surgery.
Next Steps
kindly take opinion from Orthopedician specializing in  ACL repairs for the same.

The need for ACL surgery depends on what problems your brother is getting with the knee and how much is it affecting his life. If this is just after injury, it is advised to rest the knee to settle the inflammation down. Then work on improving the quadriceps and hamstrings for 3-4months. Even after that if there is feeling of knee giving way and there is difficulty is doing sporting activity ACL surgery is recommended. Surgery when done in a good center has got good results. Rehabilitation after the surgery is very crucial.

Hello there....well u need to share the MRI report of your brother to know the exact status of the ACL injury....as grade 3 and some grade 2 injuries require surgery...while grade1 and some grade 2 can be managed conservatively through braces and extensive physiotherapy protocol....hence its important to know the extent of injury.
Next Steps
ACL repairs are arthroscopic surgeries which are done very professionally and its not a major operation as such....your brother will be discharged in 2 to 3 days on the advice of treating orthopaedic surgeon.
Health Tips
even if you opt for surgery or without...your brother needs to undergo a strict regime of ACL physiotherapy protocol. Regards & take care.

Nowadays we do the ACL surgery using key holes. It is a very safe and has a high success rate, when done in hand of an expert arthroscopist. As the surgery is performed using key hole methods it is less painful. There is no need to worry. The patient is able to walk within a few days. Good exercises done post op will be very helpful. I have done hundreds of such cases and am talking from experience
Next Steps
Visit an arthroscopy surgeon

Hello. In a young male with MR proven or clinical ACL injury, and with symptomatic instability (feeling of knee giving way), surgical ACL reconstruction is indicated pending failure of conservative treatment. Yes, this is major surgery with a commitment from patient to undergo almost a year of rehabilitation with physiotherapy. Results however are good. Choose your surgeon carefully-someone with expertise in this area. Best wishes. Dr M Vashishtha FRCS (dr-MV.com)
