To obtain a Person With Disability (PWD) certificate, you would need to visit the local government hospital or a specialized clinic that provides disability assessment services. You would need to provide your medical records and undergo a thorough medical examination by a qualified medical professional who will assess the severity of your condition and determine if you meet the eligibility criteria for a PWD certificate.
The eligibility criteria for obtaining a PWD certificate vary from country to country, but generally, they are based on the degree of disability and the impact it has on your daily activities. The medical professional may also require additional tests or evaluations to assess your condition accurately.
Once you have been assessed, the medical professional will issue a PWD certificate if you meet the eligibility criteria. This certificate will enable you to access various government schemes and benefits designed for people with disabilities.
I would suggest that you reach out to the nearest government hospital or specialized clinic that provides disability assessment services for further information on the procedure to obtain a PWD certificate in your locality.
