Persistent high fever at night for over 6 days in a 10 year old kid. Reaches 104-105°F, admitted to hospital and given IV drip.
Dengue negative, typhidot negative, malaria negative. SGPT, SGOT and CRP slightly elevated. CBC report is completely normal.
No other symptoms except for general tiredness from fever. Is this typhoid? We have seen no improvement at all despite giving antibiotics for 5 days. Before that we were just giving paracetamol. The fever is almost cyclical, comes at 8PM and subsides only by sponging. Then early morning again. It is never below 103°F when it flares.

Persistent high-grade fever for over 6 days despite antibiotics needs a detailed clinical review. Since dengue, typhoid, and malaria are ruled out, other possibilities such as viral fever (EBV, CMV), occult bacterial infection, or early inflammatory conditions (like Kawasaki disease or autoimmune fever syndromes) must be considered. Slightly elevated SGOT/SGPT and CRP indicate ongoing inflammation but don’t confirm typhoid alone.
You should get re-evaluated urgently by a pediatrician or infectious disease specialist for repeat investigations — including blood culture, chest X-ray, urine culture, ferritin, ESR, LDH, and possibly ultrasound abdomen to rule out hidden infections or inflammatory causes.
Fever of this pattern should not be managed at home, especially if it is touching 104–105°F. Continue sponging for comfort, maintain hydration, and follow the treating doctor’s instructions strictly.
Next Steps
Please consult your pediatrician again for detailed evaluation and monitoring — persistent fever of this kind always warrants specialist review rather than home treatment.
Hello,
I understand your concern. In children, the temperature can easily shoot up high. This sounds mostly like a viral infection. But to get to correct diagnosis more detailed history and tests are needed. Please contact.
