Perianal Abscess
I had a perianal abscess which has gone from a 10 day course of antibiotics. It was swollen quiet a lot. I Showed it to a specialist and he said there is a 25 percent chance it can come back. I thought abscess always needed draining ? I am confused now. Will the abscess come back? Should I get a MRI?

A perianal abscess (small ,no much fluid collection) does not always need draining if it responds well to antibiotics and reduces in size, as in your case. However, there is indeed a chance about 25 % that it might return, especially if a fistula (a connection between the abscess and the anal canal) has formed. An MRI is helpful in detecting hidden fistulas or recurrent abscess risks. If symptoms recur, or you remain concerned, discussing an MRI with your specialist would be a good next step. It will be good if you do sitz bath 10 to 15 mins three times a day (pus enough salt in a basin and  add water and sit for 15 mins )

Yes this could recur after certain period of time. Drainage of abscess is required only if pus pointing has been formed or the swelling is fluctuating on applying two fingers gently over the swelling . This is generally elicited by surgeon Also surgeon if at all required might aspirate and if purulent discharge comes out then drainage is needed.
Next Steps
if next time this happens, do check for above things and associated fever , pain Surgeon examination is crucial

Abscess relapse is common if the root is not treated or drained completely. So MRI can be taken for best outcome.
