Hello sir... pain and sensitivity may be due to 2 reasons.. 1..Erupting 3 rd molar on both sides pushes the 2 nd molar and cause erosion of enamel and dentine of that tooth and it is approaching the puip which cause sensitivity and pain 2.The impacted lower right 1 st premolar which give pressure to the reained primary teeth also cause sensitivity and pain.. MANAGEMENT: 1..you need to go for extraction of 3 rd molar and go for filling with subbase of calcium hydroxide on right 2 nd molar and root canal treatment of left 2 nd molar as the erosion approches the pulp... 2.. removal of retained primary teeth and orthodontic eruption of permanent premolar.....

opg shows cavities  in second mandibular  molar on both side which are close to nerve inside that's the reason  for sensitivity and pain . wisdom teeth removal is necessary in lower jaw. visit a dentist for check up and treatment

It’s might be because of the permanent premolar that is trying to erupt by hitting the primary which has never shedded out . The primary tooth can be removed . The last two impacted wisdom tooth has also affected the adjacent tooth leading to caries . This can also end up in severe pain and swelling

You have an extra tooth below the premolar s so that can be one reason for pressure on your anterior teeth. Also both sides have impacted wisdom teeth n is also affecting and causing decay to the second molars. Please go to a good oral surgeon ASAP so that these problems are solved by extracting the wisdom teeth.
