Hi, please make sure to clean the tongue too everytime when he brushes his teeth.
Also brushing teeth and cleaning tongue is mandatory at night, and rinse mouth after every meal.

Hello. You need to rule out any tartar deposits in his teeth, any cavities and any tonsil stones that he may have since these can also cause bad smell. Get a dental and pharyngeal check up done by dentist and ENT respectively.

Bad breath usually bcz of three issues like poor oral hygiene, throat issue,stomach issue
If it is bcz of poor oral hygiene brush twice daily, or if deposition is thr get it cleaning done
In children, bad smell from mouth can be due to a variety of reasons apart from just teeth.
Frequent viral like colds &flus ,tonsil infection/enlargement ,stomach issues like constipation can all cause bad breath.
From teeth point of view ,inadequate oral hygiene ,cavities and inadequate water intake are the main reasons.
Best to visit dentist who can advise oral hygiene measures with proper tooth brushing technique, cleaning and fillings if required and are contributory.
Despite of brushing , some bacteria can accumulate over the teeth near gums in form of deposits contributing to bad smell. However untidy tounge can also be a culprit for bad smell .so you need to check it up clearly and assist him in proper tounge cleaning. If there are deposits on teeth you will have to get it cleaned at a dental clinic
If the mouth is clean, the cause is often from the stomach :
Poor digestion.
Worm Infestation (Very Common in Children)
Check for teeth grinding or if
Child has low appetite
Constipation might also be the cause.
