Continue Simple PCM syp 6 hourly with correct doses.15mg/kg/dose every 6hourly alongwith tepid sponging.water should not be cold or hot, just room temperature.Cold water sponging can cause rigors which can increase temperature of body.Since its second day only, possibility of viral illness is more specially if child is active playful in between fever episodes.Just keep hydrating with lots of oral fluids.Look for any danger signs like lethargy, decrease oral intake, continuous high grade fever, recurrent vomiting, decrease urine output, dry lips, fast breathing.Consult pediatrician immediately if any danger signs noticed.
