You have changes in your spine both in neck region and lower back region. Disc bulge is present in cervical spine (neck region ) causing nerve compression, that might be causing travelling pain in your arms. In your lower back ( lumbar area ) you have disc bulge as well as structural issues in your spine. Disc bulge is compressing your nerve in lower back too, that might be giving you travelling pain in your legs.
Next Steps
Its unlikely to have so much issue at your age. I would recommend you to visit spine surgeon if physiotherapy is not giving relief.

Surgery to be done or not, it depends upon the symptoms you are getting from your problem. If your symptoms are severe, then only you require to undergo a surgery otherwise with physiotherapy and with proper posture care, you will be benefited.
Next Steps
Consult physiotherapist online or off-line

Lifestyle modification required

Can you share your medical history — how did this condition progress so much? You have bulging in both your back and neck. Before going for surgery, take a few physiotherapy sessions; that will really help with pain relief and improving strength. Also share your MRI films
Next Steps
Take proper rest do not lift weight or avoid driving also work on daily life routine how to sit and walk

You don’t need surgery unless: - You have severe or progressive neurological symptoms (e.g., weakness, foot drop, bladder/bowel issues). - You’ve had no relief with 6–8 weeks of conservative treatment (physiotherapy, medications). - Severe radicular pain that is not manageable with conservative treatment.
Next Steps
Suggested Plan: - Consult a spine specialist (orthopedic/spine neurosurgeon) for clinical correlation. - Start/continue physiotherapy focused on core stabilization, posture correction, and neural mobilization. - Pain management with medications, if required. - Consider injections (like epidural or nerve root blocks) if pain is persistent and surgery is to be avoided.
