Left upper second last tooth can be saved by either filling or RCT depending on clinical examination findings . If it requires RCT then it will be followed by capping .
As per the xray no severe problem is seen in upper left 2nd molar so I don't think there's any need for extraction... whatever problem u r having pls consult it at the clinic again.
You have to get your third molar extraction done asap.. lower third as well..and after that..you should go for normal tooth x ray to figure out whether the other tooth require RCT or not as Opg does not give clear picture.. as there is chance of 20% that after extraction your pain will not go..in that case you can go for Rct
